Introduction Consumers often encounter palladium through jewellery and dental restorations. Palladium allergy frequently coexists with nickel allergy due to cross-reactivity. This study analysed palladium patch test data and case reports from 1986-2008, involving 10,778 patients tested with palladium chloride. The median prevalence of palladium allergy was 7.8% among dermatitis patients and 7.4% among dental patients. The prevalence of palladium mono-sensitisation, defined as palladium allergy without nickel allergy, was 0.2% in dermatitis patients and 0.5% in dental patients. A slight increase in mono-sensitisation prevalence was observed over the study period. Clinically relevant palladium allergy should be suspected in patients with allergic contact granulomas at piercing sites or those with palladium patch test reactivity without nickel reactivity. Palladium salts should be included in dental screening patch test series, and its use in jewellery should be limited until more is known about sensitisation risks.
